Training – Terms and Conditions



Waiver of Risk and Liability

                                                 

NOTE: You, agree to participate in “Certified Personal Fitness Training” and or "Group Fitness Training" and you are
Healthy and have no Limiting Medical Conditions or Health Risks. "Unless noted by the Trainer and Physician. The Training will only be conduction by a Certified Personal Trainer and is not under Medical Supervision.
 Participation is at your own  Risk. 
     
You agree" NOT" to pursue any liability or fault To (COMPANY)(S): CrossFit inc., CrossFit Tuff Luv, Maximumcore, or Toyota Sunnyvale for any injury or health condition that may occur from your voluntary activity in the training or class programs.
It is the Company practice to take photos and video of the training and class workouts for review of training techniques and clients training progress and also for Website and other promotional media.

 Terms: Session Purchase-
Your training will be scheduled to your requested times to the best of our ability.
Your session purchases are in packages of 8 Training sessions or 8 Group Classes
The Personal Training session times are 45min. The Group Training Classes are 45min.
(this time is approximate the workout maybe shorter or longer from day to day)
your "Training" sessions expire as follows. Without agreed notice,
If you stop your training, 45 days after your last training date the remaining sessions a Maximum of 4 sessions can only be used in the Group Training Classes, per the current class schedule.
The company makes every attempt to contact our clients to keep them on schedule.
Promotion sessions: for any company specials or marketing, these sessions must be use within 6 weeks of session purchase (i.e... if you purchase sessions with added promotion sessions) the total sessions must be used in 6 weeks. If you have not trained all the sessions within 6 weeks only the sessions remaining in the standard package of sessions will still be active. (Rev. 11/01/2011)


CANCELLATION POLICY:
CrossFit Tuff Luv – MAXIMUMCORE:
HAS A 24 HOUR TRAINING SESSION CANCELLATION POLICY -
  
WE MUST HAVE NOTICE BY 4pm THE DAY BEFORE YOUR SESSION.
PLEASE CONTACT YOUR TRAINER DIRECTLY FOR RESCHEDULING YOUR TRAINING SESSION. THE CLIENT WILL BE BILLED FOR THE SESSION FOR LATE OR NO NOTICE 
OF YOUR TRAINING SESSION.
  


Gym Use: During Gym hours- Training clients have open use to the cardio equipment in the Cardio Room “only”.  If you are stretching warming up or your cardio programs, don’t use the Gym Workout Room. The Workout room and equipment is for training and group classes only. Clients with training sessions that day have priority use of the equipment – please be courteous of this fact if you are using the equipment on your off training days.

Note: A signed copy of these Terms and Conditions are kept in the clients file.
